HOSPITAL RELATIONS

Psyclarity Health has an in-depth understanding of the necessary administrative technicalities involved in referring patients from hospitals to addiction treatment centers. Our competence around the various protocols required has made the process for referring clinical specialists and patients simple and streamlined.
Medical workers often tend to patients with drug and alcohol-related issues, such as overdoses, alcohol poisoning, DUI’s and manic breaks. After the initial hospital care, many of these cases may require more intensive treatment. In some cases, patients may approach the hospital directly, asking for advice on where to seek assistance for an acknowledged problem. These patients may have unsuccessfully attempted treatment in the past or be concerned that their environment will not be conducive to their wellness and recovery. This is where our trusted team at Psyclarity Health New Jersey comes in with their extensive experience in providing the relevant guidance to third-party referrers such as case workers, social workers, and discharge planners or nurses.

Admissions Process for referrals

The Psyclarity Health coordination team is available 24/7, to assure hospital healthcare professionals of a smooth admission process for their patients. Upon referral, a certified medical practitioner will provide a pre-admission assessment to determine the patient’s needs, where doctors will assess the patient’s medical condition, their detox needs, substance use history, and mental health history, along with any co-existing conditions.

Benefits of Outpatient Rehab

During the treatment process, referring professionals can be assured that their patients will receive expert medical attention specific to their addiction requirements. After completing detox, patients may be guided through additional therapies, such as individual and group counseling, which can be supplemented by various activities to assist with developing life skills that develop constructive coping methods without resorting to drugs or alcohol.

Dedicated Case Management

To ensure the wellness of the referred patient, a dedicated case manager oversees administrative requirements during the admission, as well as the discharge process. This helps to ensure seamless administration, and to provide ongoing reports and updates on weekly patient progress. When being discharged, communication is even more vital and the referring healthcare workers are kept up-to-date with the progress, and all relevant parties are provided with a discharge checklist. If necessary, there is also family education covering the overall program.
